Suggest an Indicator

We are collecting information and comments about our Goals and Objectives for Action. First, you will be asked to fill out a brief form so that we may learn more about you. This information will be kept strictly private.

After completing the info form, you will be asked to review the NSSP Goals and Objectives and submit possible indicators for them. You do not need to suggest indicators for all the objectives. In addition, you will be given an ID number that will allow you to return to the survey if you have not completed it.
